Appointment Types
A referral from your GP or specialist is required for consults and procedures.
- This covers medical consultations for 40 minutes and follow-ups for 20 minutes.
- Some pain procedures and all medical advice are covered by MSP.
- Average wait time to see Dr. Squire: 8-11 months
- Average wait time to see Dr. Amayem: 1-2 weeks
- Appointments must be cancelled with 48 hours notice to avoid a cancellation fee of $150.
Dr. Squire and Dr. Amayem provide evaluations for the following conditions:
- Chronic pain affecting the neck, mid or low back (including chronic pain after a fall on your tailbone)
- Chronic pain affecting the sacroiliac joints with or without instability
- Chronic pain affecting the shoulders or elbows
- Chronic pain affecting the hips, knees or ankles
- Migraine or chronic neck pain with headaches
- Nerve related pain
Dr. Squire and Dr. Amayem do not perform evaluations for the following:
- Fibromyalgia, Chronic Fatigue Syndrome, or Lyme Disease (these conditions are seen and managed by the Complex Chronic Disease Program at BC Women’s Hospital)
- Review, optimization or prescribing of opioid medication

The questionnaire takes approximately 30-60 minutes to complete. Due to time constraints you will not get a comprehensive evaluation without completing this questionnaire. If you do not require a thorough evaluation then you do not need to fill this out.
If you have seen other specialists or had any tests such as nerve conduction studies, MRI, X-rays or CT scans, please make sure we have a copy before your appointment. Do not assume your family physician has sent us a copy.
- The clinic offers 1-hour private pay consultations within 2-4 weeks for non-MSP covered treatments listed below.
- The cost of the consultation is $400.
- Private treatments are billed separately.

Unsure if Prolotherapy, PRP, Botox or Fascial Manipulation is appropriate for you?
Option 1: Book a private consultation. These appointments are generally booked within 2-4 weeks. Any private pay treatments we feel might be helpful will be discussed and billed separately.
Option 2: Ask your family physician for a referral and you will be placed on the waitlist (8-11 months). The consultation regarding your pain will be covered by MSP. Any private pay treatments we feel might be helpful will be discussed and billed separately.
A private consultation is not just designed to determine if you would benefit from one of the private treatments such as PRP or Prolotherapy, but to try to determine the cause of your pain. If we think you need more investigations, this will be organized. ALL treatment options including private and MSP-covered will be discussed.
All of these treatments or any injections into joints, ligaments or tendons or around nerves are performed using ultrasound to ensure precision injections without harmful radiation.
- Platelet Rich Plasma injections ($400-$600 per treatment – usually 1-2 treatments)
- Prolotherapy injections ($200-400 per treatment – usually 2-6 treatments)
- Botox injections: The physician fee for injecting the Botox is $200. Dr. Squire does not perform cosmetic Botox injections. The cost of the Botox solution is separate and may or may not be covered by extended health or Pharmacare. Dr. Squire injects Botox for:
- Headaches with neck pain (e.g. after whiplash) or migraine
- Some kinds of nerve pain (such as post-herpetic neuralgia) or unusual muscle spasm (e.g. piriformis syndrome)
- Fascial Manipulation (Stecco technique) is a specialized type of manual/massage therapy. $100 per 20-minute treatment session.

1. Independent Medical Legal Evaluations (IME).
Dr. Squire provides a detailed biopsychosocial evaluation with:
- All known diagnoses (and reasons supporting the diagnoses)
- Detailed disability descriptions (if present) for work, education, home and social functionality including physical and psychological restrictions
- Causation including biomechanical correlation
- Integrated prognosis with diagnoses
- Detailed investigation and treatment recommendations for each diagnosis with specific MSP and non-MSP funded options
2. Medical Legal Opinions (MLO) if Dr. Squire is the treating physician
Prior to preparing this report, Dr. Squire’s office will schedule a 1-2 hour interview of the patient to provide complete and up-to-date medical information, examination and diagnostics in order to comment on causation and prognosis. The report will include:
- All known diagnoses (and reasons supporting the diagnoses)
- Detailed disability descriptions (if present) for work, education, home and social functionality including physical and psychological restrictions
- Causation including biomechanical correlation
- Integrated prognosis with diagnoses
- Detailed investigation and treatment recommendations for each diagnosis with specific MSP and non-MSP funded options
3. Expedited Chronic Pain Evaluation and/or Treatment for Insurance or Legal Purposes
This appointment can be booked by an insurance provider or lawyer who would like a rapid consultation for their client. Appointments and consult letters are usually available within 2 weeks. This is not a medical legal evaluation. Note that Dr. Squire no longer has a contract with WorkSafeBC and WorkSafe patients cannot be offered expedited appointments unless they want to pay for them themselves.
The $750 fee includes the history, physical examination, review of previous investigations, diagnoses and recommendations for treatment or additional investigations.
When a previous IME has recommended Botox injections, a specific ultrasound guided diagnostic block, prolotherapy, or PRP these can be performed in the office. The exact cost will vary with the request. Please call our office for more information.

What to Wear
Dr. Squire and Dr. Amayem will be performing a detailed physical examination that involves evaluating your whole body posture. To perform this they need to be able to see your entire body without a gown. Please bring a pair of shorts (not longer than mid-thigh) and for women, also a sports bra. If you would prefer not to have this kind of examination, a local examination of the painful area only will be conducted.

COVID-19 Information for Patients Attending the Clinic
Please read before coming to your appointment
- Do you currently have any respiratory or flu symptoms: fever, cough, sore throat or breathing difficulties?
- Have you or any of your close contacts travelled outside of Canada including the USA in the last 14 days?
- Have you knowingly been in contact with anyone who has COVID-19 or is being investigated for this?
If you answered YES to any of the three questions above, please cancel your appointment!
If you have any respiratory symptoms (colds, coughs, fever), cancel your appointment! No fees will be charged but we would like as much notice as possible.
If you only need a phone or telemedicine consultation, we will be arranging this too: please ask us.
If you are over 70 years old, you have to determine your own risk vs benefit to decide if you wish to come or not. Some provinces recommend you should stay home/self-isolate if you are over 70.
The following precautions reduce the risk of COVID-19 for everyone concerned and allow us to remain open to treat urgent pain conditions
- All patients coming to our office at this time are requested to wear a mask.
- Wash your hands just before you come.
- Hand sanitizer is available in the waiting room and a separate room to wash your hands is available for after your appointment.
- Please stay 2 metres (6 feet) away from the reception area and our receptionists.
- Please leave family and friends in the lobby on the main floor
- Our waiting room is currently configured for a maximum of 4 people due to COVID-19 regulations
- We clean all clinical surfaces in between each patient with medicated wipes
